Something is seriously wrong with Drew Cain, and Ava Jerome may be the first person in Port Charles to realize that his increasingly ruthless behavior could have a much darker explanation. Drew has become colder, more calculating, and frighteningly comfortable manipulating the people around him. But what if this transformation isn’t entirely about ambition? What if something buried inside Drew’s mind has suddenly been switched back on?

Ava may be working with Drew, but trusting him completely would never be her style. The closer their alliance becomes, the more reasons she has to investigate the man standing beside her. Drew’s unpredictable behavior could eventually convince Ava that she needs leverage, sending her digging into parts of his past that everyone else has seemingly forgotten.
That investigation could lead Ava straight into Drew’s darkest history. Years ago, Drew became the victim of memory manipulation and schemes connected to Helena Cassadine’s twisted experiments and Peter August’s machinations. Port Charles knows pieces of that nightmare, but there has always been room for another devastating revelation: What if the damage done to Drew’s mind was never completely erased?
Imagine Ava getting her hands on an old medical or intelligence file containing information about Drew’s conditioning. Most of the document could be heavily redacted, but one chilling sentence might remain visible:
“CONDITIONING STATUS: DORMANT — NOT REMOVED.”
Those four words would change everything.
Dormant does not mean destroyed. It means sleeping. And if Drew’s conditioning was merely suppressed rather than eliminated, something—or someone—could potentially wake it up again.
Ava could begin secretly studying Drew’s recent behavior and comparing it to the information contained in the file. That’s when she might notice an unsettling pattern. Drew’s most extreme personality changes may have started after one specific event. Perhaps it was a mysterious phone call, an unexpected encounter, a medical procedure, or even a seemingly meaningless phrase spoken by someone he trusted.
Suddenly, Ava would have a terrifying new question to answer. Drew didn’t simply change. Was Drew triggered?
The most explosive moment could come when Ava discovers a strange phrase or code buried inside the classified material. Unable to resist testing her theory, Ava deliberately repeats the words in Drew’s presence.
His reaction could be immediate.
Drew might suddenly freeze. His expression could go blank for several seconds as a fragmented memory flashes through his mind—a laboratory, restraints, medical equipment, Helena’s operation, or a voice giving him instructions. Then Drew could unexpectedly mention a name or location he has no conscious memory of knowing.
Ava would instantly understand what she had just discovered.
There may still be another identity buried inside Drew Cain.
This twist wouldn’t require General Hospital to erase Drew’s established history. In fact, it could build directly upon it. Drew’s past already includes kidnapping, memory mapping, manipulation, and powerful enemies who treated human minds like experimental technology. The shocking reveal would simply be that Port Charles never learned everything that was done to him.
Helena and Peter may be gone, but their work could still be haunting Drew.
Even worse, Ava’s investigation might reveal that dormant conditioning doesn’t simply reactivate itself. It requires a trigger.
And that would completely transform the mystery.
The question would no longer be, “What happened to Drew?”
It would become: “WHO ACTIVATED DREW?”
The list of possible suspects could open an enormous new storyline. Could someone inside the WSB have recovered Helena’s research? Could another Cassadine secret be coming back to haunt Port Charles? Could Sidwell know more about Drew’s past than anyone realizes? Or has an entirely new enemy discovered how to turn Drew into the perfect weapon?
Ava would also find herself holding incredibly dangerous information. If she knows the phrase capable of affecting Drew, she may possess something far more powerful than ordinary blackmail. She could potentially help him break the conditioning—or use it to control him.
And Ava Jerome has never been famous for ignoring leverage.
However, this theory doesn’t have to erase Drew’s responsibility for everything he has done. Programming could explain why certain darker parts of his personality have become more extreme without magically absolving him of every choice. Drew may still be responsible for the damage he’s caused, while someone else has been quietly pushing him toward becoming the man they needed.
The ultimate cliffhanger could come when Ava tests the trigger one final time.
Drew suddenly remembers sitting inside a dark room. A monitor flickers nearby. Someone stands behind the glass while a voice gives him instructions. Drew struggles to see the person’s face—and then the memory disappears.
Ava watches him carefully.
“What did you remember?”
Drew slowly looks at her.
“Who turned it back on.”
If Drew’s old programming has truly been reactivated, Port Charles isn’t simply dealing with a changed man anymore.
Someone may have deliberately created this version of Drew.
And whoever flipped that switch may still be controlling the game.
